02 — Understand Your Needs
We discuss the individual’s needs, preferences, existing coverage or program participation, and applicable care requirements.
03 — Develop or Coordinate the Care Arrangement
Services are coordinated according to the individual’s authorized Plan of Care, preferences, schedule, and applicable program requirements.
04 — Care Begins at Home
The client receives assistance in familiar surroundings while our team remains involved in coordinating the home-care relationship.
